Carolina Health Centers is dedicated to ensuring that everyone in our community can access our health care services. We provide care that connects people to health care with dignity and respect, no matter their circumstances. This commitment means offering equal access to quality health care for all individuals regardless of age, sex, color, ethnicity, national origin, disability, religion, or ability to pay. Carolina Health Centers receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for the organization and its covered individuals.

Personal Health Information

At Carolina Health Centers, our primary concern is the health of our patients, and we thank you for placing your confidence in us. Because your health information is personal, our entire staff is committed to protecting your information and your privacy.

Personal health information may include information created and received by Carolina Health Centers, may be in the form of written or electronic records, spoken words, and may include information about your health history, health status, symptoms, examinations, test results, diagnoses, treatments, procedures, prescriptions, related billing activity and similar types of health-related information.

If you have any questions about your personal health information, please speak with a staff member.

 

Your Rights

Empowered patients are ultimately healthier patients. We empower our patients in part by informing them of their rights involving how they access health care. Here’s a look at what we provide:

  • Free aids and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters

  • Free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages

If you need any of these services, just let us know when you are scheduling your appointment.

“Right to Receive a Good Faith Estimate of Expected Charges”

Under the No Surprises Act

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

FTCA FQHC

We are a Federally Qualified Health Center (FQHC) and we provide affordable healthcare and pharmacy services to everyone.

No one will be denied access to services due to inability to pay.

 

We offer a "Sliding Fee Scale" (SFS) that is based on family size and income. If you qualify for the SFS, you can pay less for the care you receive from us.

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
